Low Carbohydrate Diets And Heart Disease
Research shows that a moderately low-carbohydrate diet can help the heart, as long as protein and fat selections come from healthy sources.
- A 20-year prospective study of 82,802 women looked at the relationship between lower carbohydrate diets and heart disease a subsequent study looked at lower carbohydrate diets and risk of diabetes. Women who ate low-carbohydrate diets that were high in vegetable sources of fat or protein had a 30 percent lower risk of heart disease and about a 20 percent lower risk of type 2 diabetes, compared to women who ate high-carbohydrate, low-fat diets. But women who ate low-carbohydrate diets that were high in animal fats or proteins did not see any such benefits.
- More evidence of the heart benefits from a lower-carbohydrate approach comes from a randomized trial known as the Optimal Macronutrient Intake Trial for Heart Health . A healthy diet that replaced some carbohydrate with protein or fat did a better job of lowering blood pressure and bad LDL cholesterol than a healthy, higher-carbohydrate diet.
- Similarly, the small EcoAtkins weight loss trial compared a low-fat, high-carbohydrate vegetarian diet to a low-carbohydrate vegan diet that was high in vegetable protein and fat. While weight loss was similar on the two diets, study subjects who followed the low-carbohydrate EcoAtkins diet saw improvements in blood lipids and blood pressure.
Kidney Problems Can Worsen
Nutritional ketosis may bump up uric acid levels, leading to kidney stones or gout flares. One study found people with mild chronic kidney disease on the keto diet did OK with close medical supervision. But others show those who eat any diet high in red meat and low in whole grains, low-fat dairy, and fruit are 97% more likely to get kidney disease. This is partly why some dietitians advise against doing extreme low-carb diets on your own.
What Did They Find
The reviewers found that among adults with excess body weight , those following lower-carb diets for 3-8.5 months lost, on average, one kilogram more weight than those on balanced carb diets.
However, when they ensured restrictions in energy intake were the same in both groups, by providing the food or meal plans, the difference was about half a kilogram.
In longer-term weight-loss interventions lasting one to two years, the average difference in weight-loss between those on low-carb versus balanced carb diets was just under one kilogram.
The average weight lost by groups on any weight-reducing diet varied greatly across the trials from less than one kilogram in some, up to about 13kg in others.
The studies in adults with type 2 diabetes found greater initial weight loss on low-carb diets compared to balanced carb diets: 1.3kg over three to six months. However, in longer interventions that lasted between one to two years, there was no difference.
In the small group of studies that included a maintenance period at the end of the weight-loss intervention, there were no differences in weight-loss in adults either with or without type 2 diabetes.
There were no significant differences in other health measures, including blood pressure, cholesterol, blood sugar control or risk of constipation. And they found no important clinical differences in results based on the extent of participants carb restriction.

Q& a With Dr Christopher Gardner The Lead Author
Before the release of the study, we were able to ask the lead author some questions. The answers have been lightly edited for clarity.
Q: The importance of diet quality was heavily emphasized in both study groups â something not traditionally given as much attention in other long-term diet trials. Were you able to gather any data on the participantsâ healthy eating habits?
You can partially get this data from Table 2 in the paper: we report decreases in added sugar and glycemic load for both groups and a decrease in glycemic index for the low-carb group. The low-fat group increased their fiber intake, and fiber intake was actually pretty good in the low-carb group too, even though it decreased from baseline.
We have also analyzed the entire study group, using the Healthy Eating Index score . An abstract has been submitted and accepted for the American Heart Association meeting in March, and the paper should be submitted for review before then.Unfortunately, the HEI doesnât take into account some of the additional âqualityâ items we asked the participants to work on, such as cooking more at home .
The diet assessment software does provide us with an overview of âwhole grainsâ vs. ârefined grainsâ and of âservings of vegetables and fruitsâ .We were not able to capture data like organic, grass-fed, pasture-raised, wild-caught, even though we promoted those qualities to the participants who could afford them .
